Completing your new patient forms in advance helps us prepare for your visit and spend more time focused on your care. These forms allow us to review your medical history, dental concerns, and referral information before you arrive.

Providing detailed information ahead of time allows us to make your first appointment more efficient and informative. If you prefer, paper forms are also available in the office.

Bounds Periodontics + Dental Implants is a fee-for-service specialty practice. We are not in-network with any insurance companies. This allows us to recommend treatment based on what will truly resolve your periodontal condition and create long-term stability, not what an insurance plan may or may not cover.

However, we do accept all PPO insurance plans and, as a courtesy, will submit claims on your behalf. Our team is happy to help you understand your benefits and provide estimates before treatment.
